Texting consent form
Consent to receive safety alerts by text.
A school district that runs Halyard asks each staff member who will receive safety alerts by text to sign this form when they are enrolled. The signed form is the consent record: the district keeps it and records the consent in Halyard. Print this page as it is, or copy its words onto the district's own form unchanged.
The form
Consent to receive safety alerts by text Halyard, on behalf of (school district)
By signing this form and providing my mobile number, I agree to receive text messages from Halyard on behalf of my school district: lockdown and all-clear notices, drills, and roll-call requests I may reply to. Message frequency varies with the district's emergencies and drills; most months there are none. Message and data rates may apply. I can reply STOP to any message to stop receiving texts, and HELP for help. Consent is not a condition of employment or of any purchase. My mobile information will not be sold or shared with third parties for promotional or marketing purposes. How my number is handled: halyardsafety.us/privacy. The program's terms: halyardsafety.us/terms.
For the district's Halyard administrator
Once the form is signed, open the person's entry in Halyard and turn on Reach them by text message; the same disclosure appears beside that control. Keep the signed form: it is the consent record. Someone who has replied STOP receives nothing until they reply START, whatever is set in Halyard. The terms describe the program and the privacy policy says how numbers are handled.
